Be Prepared - Here Are Things Needed to Complete the Enrollment Process:

After completing an Interview with a Director and receiving official acceptance into the program, other items will be required including:

• Must be 18 years or older

Must have proof of a high school diploma, GED, or College Transcript

Copy of your Drivers License or State Issued ID

Social Security Card

Must be able to pass a fingerprint -national background check with no felony record.

Signed Physician’s Statement (state requirement) assuring applicant’s health to take the course.

Signed Handbook Acknowledgement (we review the handbook together after acceptance into the program)

Signed Enrollment Agreement

Signed Promissory Note with agreed upon payment schedule OR if on scholarship, a signed Education to Employment Agreement.
